Job Description

Reports to: Director of Nursing Join our compassionate team as a Full-Time Pediatric Home Health Licensed Practical Nurse (LPN) and make a meaningful impact in the lives of children and their families. In this role, you'll provide comprehensive care to our young patients while empowering families through education and support. Key Responsibilities: Deliver high-quality nursing care to pediatric patients in a home setting. Assess and monitor the physical and emotional well-being of patients, home safety, and family dynamics. Educate families about their child's rights, reimbursement processes, and available care options. Continuously evaluate patient progress, collaborating with healthcare professionals to adjust care plans as needed. Administer prescribed medications and IV solutions with a focus on safety and best practices. Supervise Home Health Aides to ensure an excellent standard of care. Assist patients with daily living activities to enhance their independence and quality of life. Maintain accurate clinical documentation and record-keeping. Participate in on-call rotations to provide necessary patient care coverage. Take on additional responsibilities as directed by management to optimize patient care services. Supervisory Duties: Oversee nursing staff to ensure effective patient care through supervision, coordination, and evaluation. Engage in the recruitment, training, performance evaluation, and resolution of employee concerns. Qualifications: Current LPN license in good standing. Graduate of an accredited nursing program. Preferred: At least one year of nursing experience, but new graduates are encouraged to apply. Skills and Competencies: Excellent nursing, observational, communication, and supervisory skills. Familiarity with home health care practices and regulations. Commitment to maintaining physical and mental wellness. Empathetic, patient, and caring attitude towards patients and their families. Ability to effectively manage practical challenges and take initiative. Physical Requirements: Moderate physical activity, including driving to patient homes and assisting with mobility. Ability to use hands, communicate clearly, and stand or walk for prolonged periods. Capability to lift or move items weighing over 100 pounds occasionally. Visual acuity for close vision, color differentiation, peripheral awareness, and depth perception.
