Are you a compassionate and organized professional passionate about guiding patients through their healthcare journey? We're seeking a skilled Case Manager to coordinate seamless transitions, advocate for patient needs, and optimize care outcomes. What You'll Do: Coordinate comprehensive discharge plans for patients, ensuring smooth transitions to appropriate next levels of care (e.g., TCU, Rehab, Long-Term Acute Care). Identify and address complex discharge planning needs, proactively problem-solving to secure necessary resources and support. Effectively manage the utilization of internal resources, balancing patient needs with cost-effective care. Act as a patient advocate, ensuring their rights to self-determination are respected while facilitating access to essential services. Collaborate closely with patients, families, physicians, and multidisciplinary teams to develop holistic and personalized care plans. Actively participate in daily discharge team meetings, contributing to a unified approach to patient care. Job RequirementsWhat We're Looking For Required Qualifications:Nursing degree from an approved school of nursing. Baccalaureate nursing degree preferred. Licensed to practice registered nursing in the state of ND or in a compact state.2-3 years of clinical experience, predominately in medical/surgical nursing. Preferred Qualifications:Case Management, discharge planning or utilization review experience preferred.
