CareBridge Nurse Practitioner - Virtual Position $5,000 Sign On Bonus Available! We are actively seeking skilled Nurse Practitioners with an unrestricted Family or Adult Nurse Practitioner license in Virginia to join our team at CareBridge Health. This virtual role provides the flexibility to work from home full-time while participating in essential in-person training sessions to enhance your onboarding experience. Key Work Shift: Monday - Friday, 8:00 am to 5:00 pm (EST or CST) with rotating on-call duties. As a Nurse Practitioner, you will: Provide primary and urgent care to patients utilizing phone and telehealth platforms, focused on individuals receiving home and community-based services. Collaborate with an interdisciplinary team to create and execute tailored plans of care for adults with complex and chronic conditions. Conduct comprehensive evaluations and diagnostics to develop appropriate treatment strategies based on patient goals. Identify care gaps and implement solutions to enhance patient outcomes. Support both patient and family needs with a holistic care approach. Educate patients and families on medication management, disease progression, nutrition, and preventive strategies. Coordinate care by communicating with other healthcare professionals regarding each patient's treatment and progress. Maintain accurate medical records following regulations and company policies. Participate in ongoing professional development as required by your certification and licensure. Prescribe medications within the limits of your state authority. Minimum Requirements: Master's degree in Nursing. Current national NP certification. Valid, unrestricted Family or Adult Nurse Practitioner license in Virginia is required. At least 2 years of experience managing complex care cases. Experience with Electronic Medical Records (EMR) is necessary. Preferred Skills and Qualifications: Active RN Compact license is highly preferred. Possession of DEA registration or eligibility is preferred. Active Virginia Medicaid license is highly preferred. Experience supporting patients with developmental disabilities and chronic illnesses is a strong advantage.
