Connect Care. Guide Families. Make Every Day Count. We are on the lookout for a dedicated and empathetic Hospice Clinical Liaison RN to join our compassionate team. In this vital role, you will facilitate smooth intake and referral processes, ensuring that the needs of patients, families, and healthcare providers are met efficiently. You will also contribute to creating a greater understanding of hospice services within the community. Key Responsibilities Manage incoming referrals from hospitals, physicians, skilled nursing facilities, and other healthcare professionals Evaluate patient eligibility for hospice services through medical record reviews and direct communication with families and medical teams Provide education to patients and families regarding hospice care, offering clarity and reassurance during their decision-making process Ensure all necessary documentation is completed and that the admission process adheres to legal, clinical, and payer requirements Act as the primary link between referral sources and our hospice care team to ensure effective communication and prompt responsiveness Maintain visibility and accessibility by visiting hospitals and other facilities regularly, working closely with discharge planners and case managers Foster and nurture relationships with referral partners and identify outreach opportunities to enhance awareness of hospice services Your Qualifications Education Current RN license with three to five years of hospice experience required. BSN preferred with five or more years of hospice experience. Experience At least three years of hospice experience is required. Previous roles in discharge planning, case management, or education are a plus. Licenses and Certifications Active RN license in your state of employment. CPR certification required. Must possess a valid driver's license and automobile insurance. Skills and Knowledge Strong understanding of hospice care and the specific needs of patients and families. Familiar with community resources and referral networks. Excellent assessment and documentation skills. Capable of supervising staff, coordinating services, and managing programs. Exceptional communication skills, with sensitivity to diverse backgrounds. Personal Attributes Demonstrates compassion toward terminally ill patients and their families. Flexible and capable of managing multiple tasks and priorities. Professionally mature with a collaborative attitude when working with interdisciplinary teams. Able to influence and educate referral sources, patients, and families effectively. Showcasing a commitment to quality care and patient advocacy.
