That means you'll get genuine support, realistic caseloads, and a culture that values your expertise every step of the way. Your Role Provide skilled, one-on-one nursing care in patients' homes. Complete thorough OASIS assessments and design individualized care plans. Monitor progress, document accurately, and coordinate with the interdisciplinary team. Educate patients and families on care, medications, and self-management strategies. Collaborate with physicians, therapists, and aides for seamless care delivery. Participate in case conferences and quality improvement initiatives.
