Registered Nurse Case Manager (RNCM) Manageable caseloads, flexibility, and meaningful hospice work that allows you to focus on quality patient care. The Registered Nurse Case Manager supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team. Schedule, Hours & Territory • Standard operating hours are Monday through Friday, 8:30 AM to 4:30 PM • Territory: Michigan - Alma, Bay City, Midland, Mt. Pleasant, Saginaw • RN Case Managers may participate in an on-call rotation with additional pay • Some branches offer dedicated on-call staff, resulting in backup on-call coverage only What We Offer • Strong interdisciplinary team collaboration • Supportive clinical leadership and resources • Clear expectations and consistent workflows • Environment focused on quality, compliance, and patient-centered care Benefits • Competitive salary and mileage reimbursement per company policy • Medical, dental, and vision coverage • Annual employer contribution into HSA (Health Savings Account) up to $500 for individual coverage and $1,000 for two-person or family coverage • Company-paid Life and AD&D • Voluntary disability, accident, critical illness, and hospital indemnity options • Employee Assistance Program, WillPrep, and travel assistance resources • Tuition reimbursement • Paid time off and paid holidays • 401(k) with employer match at 100 percent on the first 3% and 50 percent on the next 2% Key Responsibilities • Conduct comprehensive patient assessments and develop individualized plans of care • Provide skilled nursing care in accordance with hospice standards and physician orders • Coordinate care with physicians, caregivers, and interdisciplinary team members • Educate patients and families on symptom management, disease progression, and end-of-life care • Document assessments, interventions, and care plans accurately and timely in the EMR - Homecare Homebase (HCHB) • Participate in interdisciplinary team meetings and care planning • Support after-hours patient needs as part of an on-call rotation when applicable Professional Experience/Qualifications: Maintains the agency's mission, philosophy and core values. Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management. Always maintains patient confidentiality, including all HIPAA regulations. Regulatory Requirements Licensed or registered in the state in which currently practices Must pass a criminal background check & MVR check. Completed health statement acknowledging ability to perform the duties of the position. Valid state drivers license. Proof of current automobile insurance. Current CPR card; TB testing per agency policy;
