What you'll do in this role:Hospice patient identification, follow-up, and management in the hospitalManage Hospice referrals for assigned area, which may come from several sources including but not limited to: Hospitals, Discharge Planners, PhysiciansWork closely with the insurance departmentReview charts to determine eligibility for program(s)Determine Hospice level of care needs for each referralCall/ conduct visits with patients/caregivers to explain and discuss Hospice servicesProcess appropriate documents including physician orders/certificationCommunicate with Hospice Supervisor/Manager regarding status of referral to facilitate continued admission process and transition of care as needed by the patient We are looking for compassionate Hospice Admissions Registered Nurse with:RN license in the state you workAssociate degree or higher from an accredited School of NursingTwo years of RN experience, hospice experience preferredCurrent CPR and BCLS certificationCurrent driver's license We are an equal opportunity employer and value diversity at our company.
