Nurse Practitioners / Physician Assistants work in outpatient settings as part of a collaborative team that includes physicians, PAs, medical assistants, scribes, and care managers. With support from specialists and hospital partners for acute and specialty care, providers focus on primary care, care coordination, and population health within the clinic. Core Responsibilities: Deliver high-quality primary care for routine and acute visits Perform annual wellness visits and health risk assessments Coordinate care with specialists and other providers Support population health initiatives and preventive screenings Assist with phone triage and patient outreach Educate patients on conditions and treatment plans Participate in community outreach and promotional activities Conduct home visits as needed Perform other assigned duties Required Qualifications: Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate National certification in at least one of the following specialties: Family Nurse Practitioner Adult-Gerontology Primary Care Nurse Practitioner Adult Nurse Practitioner Gerontological Nurse Practitioner Active, non-probationary state Nurse Practitioner license Active DEA license US Work Authorization Preferred Qualifications Experience in primary care: internal medicine, geriatrics, or family medicine Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs). Passion for teamwork and the opportunity to collaborate cross-functionally Desires to be a part of an innovative model focused on empirically-guided population health Bilingual proficiency in applicable areas
