Position Details: Work Shift: Monday - Friday, 8:00 am to 5:00 pm CST with rotating on-call responsibilities. As a Nurse Practitioner, you will collaborate with our physicians, patients, and their families to create comprehensive care plans tailored to individual health needs and aspirations. You will provide crucial clinical and non-clinical support to ensure patient well-being. Key Responsibilities: Deliver primary and urgent healthcare to patients via telephonic and video consultations, which assist those receiving home and community-based services within state Medicaid programs. Craft and execute clinical care plans addressing complex issues for adult patients, including co-morbid health conditions and limited resources. Conduct thorough patient assessments, including history taking and physical exams, to develop effective and individualized treatment plans. Identify care gaps and implement strategies to close them. Meet the physical and psychosocial needs of patients and their families, while collaborating with a multidisciplinary team. Educate patients and families on medications, managing chronic illnesses, dietary needs, and prevention strategies. Coordinate with other healthcare providers and maintain documented medical records in compliance with regulations and standards. Commit to continuing education as mandated by certification requirements. Prescribe medications in alignment with state authority. Minimum Qualifications: Master's Degree in Nursing. Current national NP certification. Active Family or Adult NP license in Texas. Valid RN Compact license. 2+ years of experience in managing complex patient cases. Familiarity with Electronic Medical Records (EMR). Preferred Qualifications: DEA registration or eligibility. Active Medicaid number in Texas is highly preferred.
