Role Overview: Support patients, families, and loved ones through their end-of-life journey with compassion and care. Conduct comprehensive assessments to set goals for symptom management and end-of-life care using effective case management strategies. Collaborate as part of an interdisciplinary team to address the holistic needs of patients. Develop and sustain therapeutic relationships through effective communication with patients and their families. Utilize cutting-edge technology to document skilled services provided. Qualifications: Registered Nurse with a valid Connecticut RN license. At least one year of RN experience, preferably in homecare or hospice. Valid driver's license and reliable transportation are required. Computer proficiency is essential.
