As a Hospice Visiting Nurse (RN), you will: Provide essential support to patients and their families on their end-of-life journey. Conduct comprehensive assessments to effectively manage symptoms and set goals for end-of-life care. Work collaboratively as part of an interdisciplinary team that addresses the holistic needs of patients. Develop and maintain strong therapeutic relationships through effective communication with patients and their loved ones. Employ cutting-edge technology for precise documentation of skilled services rendered. Qualifications: Current Registered Nurse with a valid CT RN license. At least one year of RN experience; homecare or hospice experience is a plus. Valid driver's license and reliable transportation are required. Proficiency in computer usage is necessary.
