LTSS Service Coordinator - Registered Nurse Schedule: Monday-Friday 8am-5pm EST Location: Candidates must be located in White County Field: This is a dynamic field-based position allowing you to engage directly with clients at their locations. You will travel to client sites, with occasional office attendance for key meetings and training. This flexible arrangement promotes responsiveness to client needs and ensures hands-on involvement. The LTSS Service Coordinator - Registered Nurse is crucial in managing member cases within the nursing scope of practice. You will develop, monitor, evaluate, and adjust individual care plans to reflect the needs of each member, emphasizing person-centered care and optimizing health outcomes across the care continuum. Your Impact: Conduct both telephonic and in-person functional assessments to identify and evaluate member needs, covering physical health, behavioral health, social services, and long-term services and supports. Detect high-risk complications and coordinate care with both the member and healthcare teams. Oversee members with chronic conditions and disabilities to ensure optimal usage of health benefits in a cost-effective manner. Collect comprehensive member histories to develop tailored individual care plans. Collaborate with members, caregivers, families, and healthcare providers to establish short and long-term goals, identifying those who might benefit from additional care options or programs. As the RN, you will develop and manage care plans for services and facilitate members' access to these services. Assist in implementing care plans by coordinating authorizations/referrals within benefit structures, ensuring effective utilization of relevant services. Engage with Medical Directors, Physician Advisors, and Inter-Disciplinary Teams on person-centered care plan management and assist in addressing provider-related issues. Minimum Requirements: A high school diploma or GED and a minimum of 3 years’ experience with individuals with chronic illnesses or disabilities in a Service Coordinator, Case Management, or similar role, or an equivalent combination of education and experience. Current, unrestricted RN license in applicable states. Preferred Skills and Experience: BA/BS in Health Sciences or Nursing. Preferred experience in case management with older adults or individuals with disabilities.
