The pharmacist works directly with patients, providers, and care teams to identify medication"‘related issues, optimize therapy, ensure adherence, and promote safe, evidence"‘based medication use.Responsibilities: Provide Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews (TMRs) via phone, telehealth, or in"‘person. Identify and resolve medication-related problems, including efficacy, safety, adherence, cost, contraindications, and duplications. Provide patient education on medication therapy, disease states, lifestyle modifications, and self-management strategies. Develop individualized medication action plans and share follow-up recommendations with providers. Ensure appropriate documentation in MTM platforms (e.g., OutcomesMTM, Mirixa/DecisionRx, internal EHR). Support transitions of care to prevent readmissions and improve continuity. Qualifications/Requirements:Experience: 1–3 years of experience in ambulatory care, community pharmacy, MTM, or clinical pharmacy practice. Strong knowledge of chronic disease management, guidelines, and MTM platforms.
