LMSW rate: $43 per hour.LCSW rate: $46 per -Person Patient Evaluation: $60 per visit (Telehealth: $40/visit). Pediatric Specialty: $69 per visit (Telehealth: $40/visit). We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life this position, you will provide psychosocial support and Social Work services to Hospice patients and their family members who are at an inpatient unit, nursing home or home care settingYou will provide pre and post bereavement servicesYou will serve as a social work member of hospice IDG and as liaison between hospice, inpatient and nursing home staffYou will develop the social work plan of care, set goals and make recommendations based on the patient and family needsYou will maintain accurate and current documentation reflecting palliative social work assessments andinterventionsTo take part in discharge planning when appropriate for patients admitted into inpatient facilityMaster Degree from the an accredited school of social workMinimum of one year experience in Social Work in a healthcare settingKnowledge of Word and excel preferredLicensed Master Social Worker (LMSW or LCSW) required in the state of New YorkValid Driver license and car maybe required based on job locationAbility to work within an interdisciplinary team environmentAbility to input clinical data proficiently
