Deliver evidence-based interventions to improve mobility, strength, balance, and functional independence. Collaborate with physicians, nurses, case managers, and other therapists to coordinate care. Educate patients and families on exercises, safety, and post-discharge recommendations. Document all patient care activities accurately and timely in the electronic medical record. Participate in team meetings and contribute to quality improvement initiatives.
