As a Nurse Care Coordinator with Indiana Lakes Accountable Care Organization (ILACO) and Goshen Health, you'll be part of a mission-driven team committed to transforming care for our community's most vulnerable patients. In this highly collaborative position, you'll partner closely with primary care providers, clinic teams, patients, and their caregivers to ensure seamless care across every stage of the health journey. You'll be the steady guide supporting high'risk patients through complex care transitions, including hospital stays, ED visits, and the critical first 30 days home. This is a role for nurses who love educating, advocating, problem-solving, and building real relationships. You'll coordinate services, remove barriers, and encourage patients to confidently self-manage their health-all while working in a supportive environment that values your clinical judgment and respects your life outside of work. What You'll Do Serve as a key member of the care team, ensuring patients receive coordinated, compassionate support across the continuum of care. Guide high-risk patients through transitions such as hospitalizations, readmissions, and emergency visits. Collaborate with primary care physicians, clinical staff, and community resources to deliver a unified, effective care plan. Educate patients and caregivers, empowering them to take an active role in their health and wellness. Promote a smooth, positive experience for patients while advancing the mission, vision, and values of ILACO and Goshen Health. Position Qualifications Minimum Education Bachelor's degree. Requires an active Indiana license as a Registered Nurse. Minimum Experience Three to five years of clinical experience. Requires clinical knowledge as well as strong analytical and critical thinking skills; an understanding of population health principles; demonstrated understanding/application of research methodology, statistical analysis and understanding of medical terminology. Requires the ability to take initiative and meet objectives. Knowledge of computer use and various database software programs. Ability to adapt and be flexible with frequent change. Possesses strong organizational and interpersonal skills.
