In this relationship-focused role, you'll coordinate services across multiple levels of care, strengthen communication among key stakeholders, and help shape programs that enhance the resident experience. The Health and Wellness Navigator will evaluate residents' needs and assist them in accessing the available resources needed to ensure a seamless transition between appropriate levels of care offered at the community. The Health and Wellness Navigator will also assist residents in facilitating their wellness and healthcare needs while ensuring social integration into the community. This position is responsible for coordinating the programs designed to meet these needs and to develop future programs. The ultimate goal is to guide residents, family members and/or caregivers through successful health and wellness transitions in order to achieve the optimal level of wellbeing and appropriate level of care. The Health and Wellness Navigator will facilitate communication with all key resources and stakeholders. Coordinates resident placement across all levels of care and ensure individuals receive appropriate services to maintain optimal health. Leads care-transition processes, including interdisciplinary meetings, communication with families and providers, and coordination of admissions and discharges with external healthcare agencies. Develops, organizes, and evaluates health education programs, support groups, and resource networks to meet resident and community needs. Maintains accurate, confidential navigation records while serving as the primary liaison among residents, families, physicians, and staff during changes in condition or care needs. Education Bachelor's degree in Social Work, Human Services, Gerontology, Psychology, Public Health, or a related field OR Equivalent combination of education and experience in resident services, case management, or senior living operations Experience 3-5 years of experience in senior living, social services, case management, resident services, or care coordination Experience working with older adults, families, and interdisciplinary teams Experience navigating community resources, support services, and care transitions Prior experience in a customer-facing or relationship-based role Experience managing documentation, resident records, or service coordination systems Knowledge & Skills Strong understanding of aging-related needs, wellness programming, and supportive services Familiarity with levels of care (Independent Living, Assisted Living, Memory Care, Skilled Nursing) Ability to identify early changes in resident condition and escalate appropriately Excellent communication, interpersonal, and conflict-resolution skills Strong organizational and follow-through abilities Ability to maintain confidentiality and handle sensitive information Comfort with technology: EMR systems, Microsoft Office, and digital communication tools Ability to coordinate across departments and external providers #ZR Our vision at Atlantic Shores is "To be a neighborhood where people want live and work."
