Contributes to department, program, and service teams to assure achievement of desired outcomes. Specific duties are determined by the work setting or patient population served. Serving patients with challenging and complex conditions requires a concentration of specialized staff able to work in highly coordinated teams. Care is organized around each patient's condition, requiring staff at multiple levels to exhibit exceptional teamwork qualities, including efficient and effective communication. Adopts the behaviors required for high functioning integrated teams consistent with CARES expectations. Essential tasks may include but are not limited to: Completes an evaluation utilizing principles of family-centered care. Designs assessment consistent with goals of evaluation, provider order, patient/family input, and priorities. Obtains necessary history and background information. Selects evaluation methods, tools, and environment to obtain valid and reliable assessments specific to the patient's age and developmental, neurological, musculoskeletal, cardiopulmonary, cognitive, and behavioral status. Administers evaluation consistent with best practices and competencies. Interprets evaluations to the patient/family and seeks their assessment of the evaluation's reliability and validity. Makes recommendations for treatment, referrals, equipment, resources, and home and community-based activities. Documents and bills for evaluations based on education and guidelines provided. Preferred: Knowledge of transition from pediatric to adult health and rehabilitation care and of rehabilitation practice for adults with childhood onset disabilities. Pediatric experience strongly preferred. Experience working with adults with childhood onset disabilities strongly preferred.
