Mental Health Professional (LCSW, LMHC, LMSW, MHC-LP) $750 sign on bonus may apply! Salary posted is for a Fully licensed LCSW, LMHC ($66,770.73 - $69,565.00) Limited permit? No problem! please apply! Limited permit and LMSW salary: $59,789.34 with supervision. CTI is a time limited, evidence-based care management approach focused on continuity of care for individuals with severe mental illness during times of transition from an in-patient setting back into the community. The Mental Health Professional (MHP) will be a part of a unique project that partners Unity of House of Troy in Rensselaer County and Mohawk Opportunities in Schenectady County together to provide a multi-disciplinary team approach to supporting participants. The MHP will provide care management services as well as therapeutic intervention, psychoeducation, developing crisis intervention/management plans, skill development, and other clinical intervention as needed. Responsibilities: Provide psychoeducation and care management services to individuals across both Rensselaer County and Schenectady County CTI participants. Work as a part of a multi-disciplinary mobile care management team. Complete person-centered care planning. Conduct mental health assessments. Provide therapeutic intervention for both mental health and substance use disorders. Provide a range of treatment, rehabilitation, and support services. Develop and maintain relationships with community partners. Complete required paperwork and administrative tasks in a timely and efficient manner. Promote overall wellness and self-management. Promote a culture of inclusion and belonging. Requirements: LCSW, LMSW, or LMHC (clinical supervision provided for LMSW/ MHC-LP) and 2-4 years prior Master's level experience providing individual therapeutic counseling. Must be comfortable facilitating groups. Previous Experience working in the mental health or substance abuse field is preferred. Valid and clean NYS Driver License. Must have reliable transportation.
