Green Mountain Family Practice is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Central Vermont Medical Center in Northfield, Vermont. In this role you will step into a well-established patient panel, practice full-spectrum family medicine in a modern, integrated clinic, and support student health at nearby Norwich University-all within a close-knit team that values collaboration, continuity, and patient-centered care. What You'll Do Provide outpatient family medicine across the lifespan Manage an inherited panel of ~700 patients Deliver acute, chronic, preventive, and basic procedural care Support Norwich University student health (includes 1 weekend/year) Optional overnight home call with stipend Document in Epic Collaborate with on-site physicians, APPs, and integrated services Schedule 30 hours/week, with flexibility in how hours are structured: three 8-hour days plus 6 hours of admin time Thursdays are required as an anchor day for team coverage and continuity 6 hours/week admin time, remote optional What You Can Expect Here A team that values communication and collaboration Clear onboarding, structured mentorship, and support for new grads Long-standing patient relationships in a community-based practice An integrated clinic with PT/OT/, nutrition, and mental health on site Steady leadership with accessible practice management Consistent schedule designed to support provider stability Qualifications NP or PA with (or eligible for) VT licensure Commitment to patient-centered primary care Comfortable working on a small, collaborative care team Preferred: 2+ years of primary care experience + familiarity with Epic New grads welcome with strong family medicine rotations Benefits Relocation assistance Medical, dental, and liability coverage 403(b) retirement plan CTO + CME support Optional compensated call If you're ready to deliver high-quality primary care to the patients and families who call Central Vermont home, we encourage you to apply!
