description: Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups. requirement: Day Shift. Mon thru Fri from 0800 to 430 pm. Sometime can pickup shifts on weekend if available and the traveler wants. Must have HCHB experience and OASIS experience for home health care. 2 years experience ideal.
