VNA Care is hiring a Per Diem LPN Clinical Admission Coordinator for our Worcester Team Position Summary: Reviews and coordinates all needed clinical information prior to patient admission to ensure the perfect pathway for new patients and to ensure an exceptional customer service experience for the patient. Communicates with branches and teams regarding new patients and maintains positive, professional relationships with all internal and external customers. Experience Pay Range: $28.00 - $38.00 per hour Per Deim hours available:On-Site: Monday and Wednesdays, 8am-4:30pm; VNA Care 67 Millbrook Street, Worcester, MARemote: Tuesday, Thursday, Friday, Saturday, Sunday.Weekend and Holiday rotations available. • Minimum 2 years related experience including medical/surgical and community health and/or experience with a managed care organization. • Previous intake, liaison or discharge planning experience is preferred. • Knowledge of home care standards and practices required. • Knowledge of medical terminology and treatment modalities of patient diagnoses required.
