Citizens Memorial Hospital (CMH) is hiring a licensed Physical Therapist for our Home Health Department. CMH is made up of 34 primary and specialty clinics, 6 long-term care facilities, and 1 residential care facility. About the department:The goal of the Home Health Department is to decrease the number of hospital days and hospital visits per patient by providing skilled nursing and other services in the patient's home. Also, to instruct and assist patients and their families regarding their diagnosis, new procedures, medications, prevention of complications, control of pain, and general home care. Individualized plans of care are developed and used to assess, intervene, and treat home-bound patients who may not otherwise receive the healthcare they need due to physical, mental, or socio-economic circumstances. A typical day includes: Traveling to the homes of patients to assess conditions and treatment plan progress, and providing therapies aimed at improving mobility and recovery. You will manage your own daily schedule of appointments, creating autonomy and flexibility for you in this role. Establishes an effective plan of care using appropriate skills for evaluation and screening Develops and implements treatment plans that lead to optimal patient outcomes Communicates effectively with patients, care team members, physicians, and others Writes clear, concise, legible documentation that includes appropriate information for care planning This position is: PRN days between the hours of 8am-5pm Job Requirements: Education: Graduation from an accredited college with a Doctorate in Physical Therapy Experience: New graduates welcome; 1 year of experience preferred Skills: Patient care Licensure/Certification: Licensed by the Missouri Board of Registration for the Healing Arts Physical: Prolonged periods of standing and walking, ability to lift, push, and pull up to 50 lbs. with patient and/or equipment assistance, ability to carry up to 35 lbs.
