Join a dynamic home health therapy team providing valuable physical or occupational therapy services across varied communities in Kannapolis, NC. This contract position offers the opportunity to deliver specialized care to adult patients managing chronic diseases and orthopedic conditions, all within a supportive and collaborative environment. Key Responsibilities: Conduct an average of 6 patient visits per day, traveling extensively throughout Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy, and surrounding areas. Schedule patient visits the day before and document care using technology including EPIC Rover and Focura. Maintain compliance with professional standards and departmental protocols. Collaborate with departmental leadership, clinical managers, and peers to ensure high-quality patient care. Manage patient assessments, treatment plans, and discharge instructions effectively. Qualifications & Desired Experience: Minimum 1 year of home health therapy experience; first-time home health travelers with adequate experience are welcome. Valid state licensure as a Physical Therapist or Occupational Therapist. Current Basic Life Support (BLS) certification. Driver's license and proof of insurance required due to travel demands. Ability to navigate an average of 60-80 miles per day with mileage reimbursed weekly. Schedule & Work Environment: No weekend rotation, call, or major holiday shifts. Schedule cycles every 13 weeks with manager-approved time off (RTO). Five-day orientation to support your success. Scrub color: Grey; free parking available.
