We are currently seeking a per diem Physical Therapist for our Adult Day Care Program. You will be scheduled to work Tuesday, Wednesday and Friday from 10:00am to 2:00pm.EDUCATION AND EXPERIENCE REQUIREMENTS Current New York State Physical Therapy License required. Graduate from a physical therapy program approved by the American Physical Therapy Association or the American Medical Association. A minimum of one year experience as a physical therapist. Ability to make clinical and managerial decisions within the scope of professional practice to perform physical therapy procedures competently and independently. JOB RESPONSIBILITIES Reports the patient's initial status, therapy plans and goals, on-going status and discharge plans. Provides assessments, plans and goals. Instructs patients, families and team members regarding programs that enhance and/or facilitate care and follow-up. Complies with all regulatory criteria. Provide screen/assessments/treatments. Provides professional leadership and clinical guidance and expertise to team members. Attends regular continuing education programs. Adheres to all aspects of Customer Service, some examples include the following. Working together, showing empathy, courteous communication, respect of others.
