This role requires strong assessment skills, independent clinical judgment, and collaboration with the interdisciplinary team. ResponsibilitiesConduct comprehensive patient assessments Develop and update individualized care plans Provide skilled nursing care including wound care, medication management, and disease education Complete documentation accurately and in a timely manner Coordinate care with physicians and other healthcare professionals Educate patients and caregivers on treatment plans and safety Monitor and report changes in patient condition QualificationsActive Virginia Registered Nurse (RN) license Minimum 1 year of nursing experience (home health preferred) Strong assessment and documentation skills EMR experience preferred Valid driver's license and reliable transportation This position serves patients in Locust Grove and Orange, VA and surrounding communities.
