Location: Both On/Off Campus Employment Type: PRN Schedule: Flexible Compensation: from $65 per visit with mileage reimbursement Meadowood Home Health is a compassionate, patient-centered home health agency dedicated to helping individuals regain independence and improve quality of life in the comfort of their own homes. We serve our residents and the community with a multidisciplinary team focused on personalized, high-quality care. Position Summary We are seeking a licensed Physical Therapist to join our team on a PRN basis. In this flexible role, you will provide in-home physical therapy services to patients with a variety of conditions, including orthopedic injuries, neurological disorders, post-surgical recovery, chronic pain, and mobility limitations. As a PRN therapist, you will enjoy autonomy in scheduling while contributing to meaningful patient outcomes. Key Responsibilities Perform comprehensive initial evaluations and ongoing assessments of patients' physical function, mobility, strength, range of motion, gait, balance, and home environment Develop and implement individualized, physician-ordered treatment plans to relieve pain, restore function, prevent disability, and promote maximum independence Administer therapeutic exercises, manual therapy, modalities, gait training, transfer training, and patient/family education Instruct patients and caregivers in safe techniques, home exercise programs, use of adaptive equipment, and fall prevention strategies Monitor patient progress, modify treatment plans as needed, and collaborate closely with physicians, nurses, occupational therapists, speech therapists, and other care team members Document all evaluations, treatments, progress notes, and discharge summaries accurately and timely in accordance with agency and regulatory standards (OASIS experience a plus) Ensure compliance with state/federal regulations, agency policies, and best practices in home health care Qualifications Current, active Physical Therapist license in PA Graduate of an accredited Physical Therapy program (Doctor of Physical Therapy preferred) Minimum 1-2 years of clinical experience (home health experience strongly preferred but not required) Strong assessment, clinical reasoning, and documentation skills Excellent communication and interpersonal abilities; comfortable working independently in patients' homes Reliable transportation, valid driver's license, and current auto insurance Why Join Our PRN Team?
