Job Description The RN - Sepsis Coordinator serves as a consultant and has the responsibility for the effective implementation of evidence-based practice for patients with sepsis. This position will work closely with physicians, advanced practice professionals, nurses and other clinicians in a team approach to patient care. The Coordinator ensures the organization, development, planning, management, and evaluation of processes used to improve the overall quality of care delivery for sepsis. The position monitors data for internal and external reporting programs and leads improvement projects to ensure compliance. The RN - Sepsis Coordinator demonstrates superior communication skills with all internal and external customers and exhibits a high degree of integrity, a solid character, and sound judgment. Responsibilities Identifies opportunities to improve the identification, treatment and care coordination of the decompensating patient and facilitates process improvement activities. Assists in development and implementation of order sets, policies and procedures specific to sepsis care and maintains annual review of pertinent policies, procedures, and order sets. Evaluates care of patients and collaborates with nursing and other hospital managers/leaders/staff to establish and/or update clinical procedures and standards of care. Participates in establishing objectives, standards of care, and policies for the program through the development of evidence-based protocols. Develops, implements, and evaluates on going service programs that assure quality nursing and patient care programs consistent with hospital mission. Oversees sepsis quality improvement program which monitors and evaluates critical aspects of care. Maintains current license and knowledge of current evidence-based practice. Assesses need for personal growth and development and takes the initiative in seeking opportunities to meet this need. Works collaboratively with the Educator(s) to ensure all clinical competencies are met. Accurately identifies real/potential problems affecting the service and implements solutions with follow through and communication. Chairs the multidisciplinary Sepsis Care Improvement Team. Serves as a community liaison in areas of sepsis education. Provides community illness prevention and health outreach education programs. Collaborates with post-acute care and the physician enterprise to reduce readmissions. QualificationsMinimum Education Bachelor's Degree Nursing Required Minimum Work Experience 5 years care of acute sepsis patients experince Required Licenses and Certifications Registered Nurse Licensed State of Florida Upon Hire Required and BLS for Healthcare Providers (BLS) American Heart Association Upon Hire Required Required Skills, Knowledge and Abilities Knowledge of sepsis pathophysiology and evidence-based standards of care. Using scientific rules and methods to solve problems. Strong analytical and decision making skills. Considering the relative costs and benefits of potential actions to choose the most appropriate one. Proficiency in Microsoft applications (Word, Excel and Access), database management, and electronic medical records systems and document preparation. Knowledge of insurance and medical terminology, coding, collections, and federal / state laws governing medical compliance. Ability to document and utilize electronic health record applications. Excellent written and verbal communication skills. Strong interpersonal and team skills.
