Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a Full-time Registered Dietitian-Nutritionist (RDN)/ Certified Diabetes Care and Education Specialist (CDCES) to join our team in Hudson Valley. Summary: We are seeking a knowledgeable and compassionate RDN who is also a CDCES to provide high-quality nutrition therapy and diabetes education. This role focuses on empowering individuals and families to manage diabetes and related chronic conditions through evidence based nutrition care, education, and behavior change strategies. Responsibilities: • Provide individualized Medical Nutrition Therapy (MNT) for patient with diabetes, prediabetes, and related chronic conditions • Deliver comprehensive diabetes self-management education and support (DSMES) in individual and group settings • Assess nutritional status, develop care plans, and monitor outcomes in accordance with evidence based guidelines • Collaborate with physicians, nurses, and interdisciplinary healthcare teams to coordinate patient-centered care • Educate patients on meal planning, carbohydrate counting, insulin management, glucose monitoring and lifestyle modification • Document patient encounters, care plans, and outcomes accurately in the electronic health record • Maintain compliance with ADA, ADCES, and organizational standards and protocols • Participate in quality improvement initiatives, program development, and community education tasks as needed • Stay current with advances in nutrition science, diabetes technology, and clinical guidelines Requirements: Experience: • Minimum of 2 years clinical experience in diabetes care or chronic disease management • Experience with diabetes technologies (CGMS, insulin pumps, glucose monitoring systems) • Strong communication, counseling and motivational interviewing skills • Experience providing group education and working with diverse populations Education: • Bachelor's degree or higher in Nutrition, Dietetics, or a related field from an ACEND-accredited program • Completion of an ACEND- accredited supervised Dietetic Internship program • Current Registered Dietitian- Nutritionist (RD/RDN) credential through the Commission on Dietetic Registration (CDR) • Master's Degree in Nutrition, Dietetics, or a related field preferred • Current Certified Diabetes Care and Education Specialist (CDCES) credential through the Certification Board for Diabetes Care and Education • Active NYS CDN (Certified Dietitian-Nutritionist) Job Type: Full Time Pay: $71,847 to $89,808 About Sun River Health Sun River Health has a rich history of providing quality, affordable health care to communities in need.
