MDS Coordinator/Clinical Resource RN

Opportunity Description Job Opportunity: MDS Coordinator Nurse Location: Wayne, NJ Schedule: Monday - Friday, 9:00 AM to 5:00 PM Are you a RAC-CT certified nurse with a passion for quality care? We're looking for an experienced MDS Coordinator Nurse to manage the assessment and documentation process that supports exceptional resident outcomes. Job Duties Oversee and complete the Minimum Data Set (MDS) process in compliance with state and federal regulations. Collaborate with interdisciplinary teams to ensure accurate and timely assessments. Develop and update care plans to reflect residents' needs and goals. Serve as a resource and support for staff on MDS and resident assessment protocols. Ensure data accuracy, regulatory compliance, and adherence to quality measures. Experience & Skills Required RAC-CT certification is required (active and in good standing). Valid RN or LPN license in the state of New Jersey. Prior experience in MDS coordination and a thorough understanding of the RAI process. Exceptional organizational, analytical, and communication skills. A strong commitment to delivering quality care.
