We're seeking the right candidate for the position of Social Worker at Old Brownlee Community Care Center. In this vital role, you'll ensure that our residents receive the highest level of care while collaborating with a dedicated team, providing exceptional clinical leadership and support. What You Bring to the Table:Essential Qualifications: Conduct psychosocial assessments of patients. Develop individualized care plans based on patient needs. Provide counseling and support to patients and families. Coordinate with healthcare teams to ensure comprehensive patient care. Advocate for patients' needs and rights within the healthcare system. Assist patients in navigating the healthcare system and accessing resources. Facilitate discharge planning and continuity of care for patients. Provide crisis intervention and emergency support. Organize and lead support groups for patients and families. Educate patients and families about healthcare options and resources. Collaborate with community organizations to connect patients with additional services. Document patient interactions and maintain accurate records. Participate in multidisciplinary team meetings to discuss patient care. Assist patients with financial concerns and connect them to relevant resources. Provide bereavement counseling and support. RequirementsThe Ideal Candidate: Master's degree in social work (MSW) from an accredited institution. Licensed Clinical Social Worker (LCSW) certification. Experience in a healthcare or hospital setting. Strong knowledge of medical terminology and healthcare systems. Excellent communication and interpersonal skills. Ability to handle sensitive information with confidentiality and professionalism. Proficiency in crisis intervention techniques. Knowledge of community resources and services. Bilingual or multilingual abilities. Experience with electronic health record (EHR) systems. Bachelor's degree in social work or related field. State licensure as a social worker. Minimum of two years of experience in a social work setting. Strong assessment and diagnostic skills. Ability to work independently and as part of a team. Proficient in Microsoft Office Suite. Excellent written and verbal communication skills. Strong organizational and time management abilities. Empathy and commitment to patient advocacy. Other Qualifications Has not been excluded from participating in Medicare and Medicaid programs. Maintains eligibility to participate in Medicare and Medicaid programs throughout employment. Participate in pre-employment and annual Tuberculosis screening.
