BID DUE DATE: FIRST TO FILL Details: Oct 11-17 (a) Facility Location: 1000 Lincoln St, Fort Morgan, CO 80701 Setting: Call Trauma Level III FTE: 1 Required Procedures/Job Duties: Ortho/trauma call EMR: Epic Support Staff: None Reason for Coverage: Employed provider requesting PTA. Dealbreakers (Do Not Present): lack of availability for all dates. No ATLS. Admissions: varies Credentialing Information: Timeframe: 1 month Credentialing with other facilities? No Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less?
