As a Home Health & Hospice RN, you will: Provide skilled nursing care to patients in their homes, primarily in Plymouth and surrounding areas. Utilize your critical thinking and assessment skills to develop and implement individualized care plans. Perform a range of essential nursing skills, including wound care, Foley catheter insertion and removal, PICC line and IV dressing changes, wound vacuum dressing changes, ostomy/ileostomy management, venipunctures, and Pleurex catheter drainage. Provide LNA supervision. Work independently while being a valued member of our collaborative team. Maintain accurate and timely documentation. Participate in a rotating weekend and on-call schedule to ensure continuous patient support. Travel to patient homes in various weather conditions. What You Need to Succeed: Required: Current New Hampshire Registered Nurse (RN) license. Required: Valid Driver's License and insured vehicle. Essential: Minimum of one year of professional nursing experience. Key Skills: Ability to work independently, strong self-motivation, excellent organizational skills, and the ability to travel. Clinical Expertise: Demonstrated proficiency in wound care, Foley catheter insertion and removal, PICC line and IV dressing changes, wound vacuum dressing changes, ostomy/ileostomy management, venipunctures, and Pleurex catheter drainage. Education: Associate's degree or higher in nursing. Work Environment: On the road: Traveling to patient homes within the designated service area. Day shift with a rotating weekend and on-call supplemental schedule.
