In this role, you will work as part of an interdisciplinary team to coordinate and manage patient care in the home setting, ensuring high-quality, patient-centered outcomes. Key Responsibilities: Coordinate and manage patient care in a home health setting Conduct patient assessments and develop individualized care plans Collaborate with physicians and interdisciplinary teams Monitor patient progress and adjust care plans as needed Maintain accurate documentation using EPIC EMR Provide patient and family education regarding treatment and care plans Requirements: Minimum 2 years of Case Management experience At least one prior travel assignment required Experience in Home Health / Home Care strongly preferred Active RN license (Iowa or Compact License) BLS (AHA) certification required Travelers may float within the same discipline across local campuses if needed About 3B Healthcare Inc.
