Lauderdale, Jacksonville Service Setting: Outpatient Clinic Coverage Type: Clinical Only Coverage Period: TBD Clinical Shift Schedule: Minimum 14 patients/day (8-hour day) On-Call Shift Schedule: As applicable PATIENT INFORMATION Patient Demographics: Adults Patient Volume/Census: Minimum 14/day Phone Consults: Yes Case Mix: HIV Primary Care, Diabetes, Hypertension, COPD, Urgent Care FACILITY INFORMATION EMR System: TBD Office Equipment Available: Standard outpatient tools Specialty Backup Available: Yes Support Staff Available: Nursing, Care Managers, Front Office Reason for Coverage: Provider Support PRIVILEGES & COMPLIANCE Credentialing Timeline: TBD COMPENSATION & BENEFITS Competitive Salary Retention and Quarterly Bonus CME/Conference Time/Professional Membership Vacation Sick Leave REQUIRED PROCEDURES Primary care management for common chronic and acute conditions HIV/AIDS clinical care Review of labs, imaging, and consultant notes Medication refill authorizations JOB REQUIREMENTS Licenses: Valid medical license Certifications: DEA, BLS, AAHIVM within 12 months of hire Other Qualifications: Board Certification preferred DUTIES & RESPONSIBILITIES Provide HIV Primary Care and manage common primary care conditions Maintain productivity of 14 patients per 8-hour day Ensure timely completion of notes and charge entries Collaborate with nursing and front office for urgent care/walk-ins Respond to refill requests and patient communication in a timely manner Supervise allied professionals as applicable Participate in after-hours call coverage as needed Complete retention reports and support clinic retention efforts Maintain certification and continuing educationaa415a4b-8b21-40fc-a65c-70d2b25ca29a
