Field Care Manager (RN / LCSW) – LTSS Location: Must be in the Jacksonville/Wilmington area Duration: This is a 6-month contract with possible extension or possibility to convert directly Pay: The pay is $38-$41/hr PLUS mileage reimbursement Schedule: Monday–Friday, 8 AM – 5 PM EST Travel: Up to 80% field travel Position Overview We are seeking a Field Care Manager (RN or LCSW) to support members enrolled in the Trillium Tailored Plan under Carolina Complete Health . This role focuses on coordinating care for individuals requiring Long Term Services and Supports (LTSS) . The Care Manager will work directly with members, caregivers, providers, and community resources to ensure members receive holistic, coordinated, and cost-effective healthcare services . This is a member-facing role that includes both remote administrative work and in-person field visits . Training Requirements Mandatory 4-week training program Training schedule: Monday – Friday, 8 AM – 5 PM EST No time off allowed during training Training start date: April 6 Work Schedule Remote administrative work: 1–2 days per week Field visits: 3–4 days per week Member visits: 1–4 per day depending on travel distance Candidates must be comfortable traveling up to 2.5–2.75 hours one way for member visits when needed. Mileage and lodging (if required) are reimbursed. Required Qualifications Active RN or LCSW license Bachelor’s degree 2+ years of physical healthcare case management experience Valid driver’s license Experience with: Care management or care coordination Hospital case management Home health Discharge planning Long-term care services Proficiency with Microsoft Office and electronic medical records
