Registered Nurse Case Manager

Description of the Role:We are seeking a compassionate and dedicated Home Health Registered Nurse Case Manager to join our team on a part-time basis. In this role, you will be responsible for telephonic care management and support to patients in Allentown, PA and the surrounding counties. Responsibilities: Reviewing patient assessments and developing individualized care plans with the care teamEnsuring all skilled nursing care, including wound care, medication management, and IV therapy is provided as orderedCoordinating and communicating with healthcare team members to ensure comprehensive and effective care is providedMonitoring patient progress and adjusting care plans as neededEducating patients and their families on proper care techniques and self-managementDocumenting Transfer and Discharge Oasis as needed Requirements: Valid Registered Nurse (RN) license in the state of PennsylvaniaMinimum of 1 year of experience in home health or similar settingMinimum of 1 year of experience in a case management roleExcellent clinical and critical thinking skillsStrong communication and interpersonal skillsAbility to work independently and manage time effectivelyValid driver's license and reliable transportation Benefits: Competitive compensationFlexible scheduleOpportunity to make a meaningful impact on patients' livesSupportive and collaborative work environment About the Company:Interim Home HealthCare is a leading provider of home health services, with a focus on delivering exceptional care and support to individuals in their own homes.
