Your Role: MDS / Care Plan Coordinator As our MDS / Care Plan Coordinator, you'll be a central part of our interdisciplinary care team - ensuring residents receive care that is not only clinically sound but also deeply personalized. What You'll Be Doing: Coordinating timely and accurate MDS assessments (admission, quarterly, annual, significant change) Leading interdisciplinary Care Plan meetings Monitoring EMR documentation for accuracy and compliance Conducting in-services and training on care planning/documentation best practices Collaborating with physicians, therapy teams, and department leaders Ensuring compliance with state and federal regulations (IDPH, OBRA, JCAHO) What You Bring: Current Illinois RN License (Required) MDS experience (Required) AANAC certification (Preferred) Proficiency in EMR systems and documentation Excellent leadership, communication, and organizational skills A compassionate, resident-focused mindset Why Choose Haven?
