Job Description & Requirements Specialty: Hospice Discipline: RN Start Date: 04/05/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Title: Travel Registered Nurse – Hospice Location: Basking Ridge, New Jersey Contract Length: 13 Weeks Schedule: 37.5 Hours/Week – 11:00 AM to 7:00 PM, 5 Days/Week (Every Other Weekend Required) Key Responsibilities Perform comprehensive patient assessments to determine appropriateness for hospice care and evaluate patient progress toward care goals. Develop and implement individualized plans of care for hospice patients and their families. Ensure appropriate utilization of hospice services while maintaining visit productivity standards. Complete admission, discharge, and revisit documentation within required timeframes and perform electronic data transfers. Provide age-appropriate education to patients and families regarding disease progression, medications, and care plans. Assume case management responsibilities , including care coordination, scheduling, and prioritizing patient needs. Participate in interdisciplinary team (IDT) meetings every two weeks to review and update care plans. Conduct Home Health Aide (HHA) supervision and collaborate with care team members to ensure high-quality care delivery. Maintain a safe environment for patients and staff, following infection control protocols. Demonstrate knowledge of pharmacology and medication safety , including medication reconciliation and maintaining current medication orders. Participate in performance improvement programs, quality initiatives, and staff meetings . Support agency growth by mentoring new employees and cross-training when required. Required Skills & Experience Minimum 2 years of Registered Nurse experience in a home hospice environment . Experience working with Electronic Medical Records (EMR) systems. Strong critical thinking, clinical judgment, and communication skills . Experience with GIP (General Inpatient) hospice care in a hospital setting . Education Requirements Associate Degree in Nursing (ASN) or Diploma in Nursing with a valid RN license.
