Home Health Registered Nurse (RN) Pueblo, CO (Onsite – Pueblo proper and west coverage area) $50–$55 per hour | Paid Bi-Weekly Full-Time | Contract-to-Hire Position Overview This position is a contract assignment with the potential for permanent hire based on attendance, performance, and business needs. The Home Health Registered Nurse (RN) administers skilled nursing care to patients of all ages in their place of residence. The RN coordinates care with the interdisciplinary team, patients/families, and referring agencies, and assumes responsibility for the overall coordination of care. Schedule Monday–Friday | 8:00 AM – 5:00 PM Participation in on-call rotation required (includes weekends) Essential Responsibilities Assess home health and/or hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs. Develop, implement, document, and evaluate individualized Plans of Care (POC). Incorporate physical, psychological, social, spiritual, and cultural factors into patient care planning. Maintain accurate, up-to-date clinical documentation for all visits, contacts, and care updates. Follow infection control measures (OSHA compliance). Educate patients and families on care plans, medications, diet, home safety, and self-care practices. Supervise and provide clinical direction to HHA/STNA/LVN staff. Assess and manage pain using appropriate techniques and education. Arrange referrals to appropriate agencies as needed. Ensure compliance with third-party payer requirements. Perform on-call responsibilities as assigned. Serve as liaison between patients, families, physicians, administration, and healthcare team members. Attend interdisciplinary and team meetings. Perform other related duties as assigned.
