POSITION SUMMARY As an integral part of our team, the Advanced Nurse Practitioner (NP) plays a crucial role in enhancing the quality of life for our residents. Responsibilities include: Conducting comprehensive assessments of patients' medical and mental health needs, developing proactive care plans that address their conditions and barriers to care. Ensuring compliance with all relevant local, state, and federal regulations regarding the clinical services provided. Prescribing medications and ordering laboratory work, diagnostic tests, and consultations as necessary. Monitoring patient compliance and response to treatment, making adjustments to care plans as needed. Collaborating with RN case managers in a team-oriented environment to deliver integrated care. WORK EXPERIENCE, CREDENTIALS, AND EDUCATION Required: FNP, AGNP, AHACNP, or PA license. Degree from an accredited APRN or PA program. Minimum of 3 years of clinical nursing or rehabilitation experience in geriatric populations. Experience with Electronic Health Records. Proficiency in Microsoft applications, including Word, Outlook, and Excel. SUPERVISORY RESPONSIBILITIES May involve providing training and guidance to facility staff as needed.
