Description: Position Summary: We are seeking a dedicated Registered Nurse Case Coordinator who will play a crucial role in assessing, planning, implementing, coordinating, monitoring, and evaluating care options for clients. This position focuses on promoting quality, cost-effective health outcomes through effective communication and resource utilization. The RN will develop and execute a personalized Plan of Care, ensuring clients receive tailored services that enable them to achieve their health goals. Strong communication skills are essential for facilitating coordination of care. Essential Duties & Responsibilities: Perform client admissions, resumption of care, and follow-up using OASIS data collection accurately and promptly. Monitor and manage core measurements to ensure compliance with department standards and operational plans. Identify and report changes in client health status to the physician and Clinical Team Manager, facilitating necessary adjustments to care. Create and develop a comprehensive Plan of Care based on clinical assessments and established guidelines, setting measurable goals and outcomes. Administer medications and treatments as prescribed in the Plan of Treatment. Provide educational support to clients and caregivers tailored to their understanding and needs, accurately documented in records. Coordinate care by collaborating with other healthcare professionals, participating in interdisciplinary meetings as needed. Document client interactions and treatment in accordance with agency policies, maintaining timely and accurate records. Communicate and plan effectively for client discharge, ensuring all summaries are completed per policy. Supervise LPNs and Home Health Aides to ensure adherence to the Plan of Care. Conduct supervisory visits at least every 14 days to assess care quality. Utilize knowledge of nursing, social sciences, and case management guidelines to adapt care plans for positive outcomes. Participate in on-call duties as necessary to support agency needs. Uphold safety protocols in compliance with OSHA, JCAHO/CHAP, and state and federal standards. Effectively operate medical equipment required for client care. Stay informed of the latest nursing practices and agency protocols through ongoing education and training. Guide and manage direct reports, ensuring effective teamwork and performance standards are met. Maintain communication with management regarding any issues or concerns that arise. Have a thorough understanding of the agency's services and protocols. Participate in interdisciplinary conferences as necessary. Submit documentation timely according to agency guidelines. Understand the organization’s operational structure and appropriate line of communication. Maintain a professional demeanor and utilize proper phone etiquette. Foster a welcoming atmosphere for clients and visitors while ensuring confidentiality. Engage in accreditation activities as required. Perform additional tasks as assigned. Licenses, Certifications and/or Registrations: Current RN license. Current CPR certification. Valid driver's license and auto insurance. Completion of 25 CEUs bi-annually. Working Conditions: Work will be performed in an office or home environment, with potential exposure to biological hazards.
