About the Role As a Nurse Practitioner at Matrix, you will engage in meaningful health assessments during visits that can last up to one hour. You will review a member's health history, medication adherence, and social environment. Our Comprehensive Health Assessment (CHA) method helps us identify and close several care gaps in just one visit. Your responsibilities will include: Conducting thorough adult and geriatric health assessments, including medical history review, diagnosis, and treatment plans. Administering and ordering necessary point-of-care and lab tests. Collaborating with physicians, social workers, family members, and caregivers to ensure comprehensive care. Providing educational resources and follow-up care in conjunction with the Primary Care Physician (PCP). Delivering services at members' residences, whether in their homes or in Skilled Nursing Facilities. Benefits of Working at Matrix Flexibility: Choose your own schedule! We conduct assessments from Sunday to Saturday between 8 am and 7 pm. Competitive Pay: Income is based on the number of completed assessments, with potential earnings ranging from $1,800 to $11,500 monthly, depending on your availability and location. Opportunity for Full-time Roles: PRN providers can transition to full-time positions and receive a sign-on bonus. Qualifications Master's Degree or equivalent experience and successful NP licensure. Current NP licensure in your state of practice, including prescription authority or eligibility to obtain it. AANP, AACN, or ANCC board certification in relevant specialties. Current BLS, ACLS, or CPR certification. At least one year of NP experience preferred; new graduates are encouraged to apply! A valid state driver's license and automobile insurance as required by your state. Strong computer skills and proficiency with medical record software.
