Job Description

The Terraces at Los Altos, a distinguished HumanGood life plan community, is looking for a dedicated Charge Nurse RN-Admissions to join our skilled nursing team. In this vital role, you will be responsible for addressing the nursing needs of our residents while embracing our Philosophy for Person-Directed Care. Full-time$45-$57/hr. Schedule: Sunday - Thursday - 11 AM - 7 PM Qualifications for Success: Current RN license 1-2 years of experience as an Admissions RN At least 1 year of experience supervising RNs, LVNs, and CNAs Ability and willingness to work on the floor as needed Current CPR certification Previous nursing experience in senior care, skilled nursing, post-acute, or sub-acute care is preferred Key Responsibilities: Provide comprehensive nursing care while involving residents in decision-making. Encourage residents’ participation in daily routines, including hygiene and mobility. Assess and evaluate residents at admission and throughout their stay to promote independence within safety regulations. Lead and guide care partners, offering support and direction. Monitor resident conditions, perform assessments, and communicate significant changes to relevant team members and physicians. Respond to emergencies, implement physician orders, and document interventions and outcomes. Administer medications, treatments, and nursing procedures accurately. Ensure proper documentation, including consents, care plans, and medication records. Engage in community QAPI programs and participate in resident care planning with interdisciplinary teams. Build authentic relationships with residents through thoughtful discussion and care. Contribute to creating a welcoming and home-like environment. Participate in training and meetings to continuously improve service quality. Foster a culture of respect and advocacy for residents' rights. Perform additional duties as required.
