Coordinate multidisciplinary services across inpatient, outpatient, and community settings to ensure continuity of care. Review clinical information, monitor patient progress, and collaborate with physicians, therapists, social workers, and insurance providers to remove barriers to treatment. Educate patients and families on treatment plans, follow-up care, and available resources to promote long-term health and stability. Unit Details: Care coordination role supporting patient-centered care delivery. Work closely with interdisciplinary teams to manage complex medical and social needs while ensuring appropriate utilization of healthcare resources.
