Nurse Case Manager II

As a Nurse Case Manager II, you will play a vital role in managing care for members with complex and chronic health needs. This position allows for virtual, full-time work while still promoting essential training and development through occasional in-person sessions. Your flexibility and autonomy in this role will enhance productivity and work-life integration. Work Hours: 8-hour shift within 8am - 6pm PST. Important Note: Associates in this role working from a California location are eligible for overtime pay according to California employment law. Your key responsibilities will include: Ensuring members have access to services tailored to their health needs. Assessing individual needs to create a specialized care management plan aimed at achieving set objectives and goals. Implementing care plans through authorizations and referrals as necessary within benefit structures. Coordinating internal and external resources to address identified needs. Monitoring and evaluating the effectiveness of care management plans and making modifications as necessary. Collaborating with Medical Directors and Physician Advisors to develop care management treatment plans. Assisting in resolving issues related to providers, claims, or services. Contributing to the development of utilization and care management policies and procedures. Minimum Requirements: A Bachelor’s degree in a health-related field with a minimum of 5 years of clinical experience; or an equivalent combination of education and experience. A current and unrestricted RN license in California. Preferred Qualifications: Experience as a Case Manager in a hospital or managed care setting. Clinical experience with various chronic diseases and medical needs. Knowledge or experience with discharge planning. Certification as a Case Manager. Your efforts will be compensated with a competitive salary ranging from $79,464 to $115,920, along with a comprehensive benefits package including paid time off, medical, dental, vision, and more.
