Position SummaryThe Patient Access Referral Coordinator plays an important role in supporting patient care by coordinating referrals and assisting patients with appointment scheduling and follow-up. This position works closely with providers, patients, and specialty offices to ensure referrals are processed accurately and efficiently while helping close gaps in care through proactive outreach.The ideal candidate is highly organized, detail-oriented, and committed to delivering excellent patient service while supporting a collaborative healthcare team. Key ResponsibilitiesReferral Coordination Review referral orders daily within the Electronic Medical Record (EMR) system Verify patient insurance information and obtain insurance authorizations when required Process referrals according to established best practice guidelines and include appropriate clinical documentation Communicate referral details with patients as needed Coordinate with providers and patients to identify appropriate specialists Assist patients in addressing potential barriers to attending specialty appointments Monitor referral tracking systems and update referral status when completed Appointment Coordination & Follow-Up Utilize appointment confirmation tools to ensure patient appointments are confirmed in a timely manner Contact patients who have not confirmed appointments electronically Assist patients who request to cancel or reschedule appointments Follow up with patients who miss scheduled appointments and assist with rescheduling Manage appointment changes due to provider schedule adjustments such as PTO, illness, or emergencies Create nursing home appointments and documentation within the EMR to support charge capture when needed EMR & Administrative Responsibilities Accurately maintain patient records within the Electronic Medical Record system Ensure patient accounts are properly created and prevent duplicate records Manage daily worklists including referrals, waitlists, and missed appointments to ensure timely follow-up and patient care coordination Qualifications Prior experience working in a healthcare setting preferred Knowledge of medical terminology preferred Strong interpersonal and communication skills Excellent attention to detail and organizational skills Demonstrated computer proficiency and experience working with electronic records systems preferred Knowledge, Skills & Abilities Ability to manage multiple tasks in a fast-paced healthcare environment Strong problem-solving and customer service skills Ability to maintain patient confidentiality and follow HIPAA guidelines Clear and professional communication skills Work Environment & Physical Requirements Primarily office-based environment Occasional light lifting up to 25 pounds Ability to sit, stand, and perform repetitive tasks throughout the workday Why Join Our Team Collaborative and supportive work environment Opportunity to make a meaningful impact on patient care Training and professional development opportunities $18-$24/hour
