Float LPNTransitions LifeCare is seeking a full-time Float LPN to join our team! This position provides routine, PRN, and crisis care for hospice patients under the supervision of the assigned RNCM and/or IDG Team Leader. The Float LPN also offers phone support to patients and families, coordinates with the interdisciplinary team, and performs office duties. The role offers a Monday-Friday 8:00 AM – 5:00 PM schedule. Qualifying candidates with at least 2 years of clinical experience may be eligible for up to a $5,000 sign-on bonus. Hiring for the Following Counties:Wake Durham Johnston Franklin Harnett Orange Chatham ResponsibilitiesProvide and document safe, competent, compassionate hospice nursing care for assigned patients under RN or physician supervision. Offer telephone support to patients, families, physicians, and partners, assisting with immediate needs within the LPN scope of practice. Review supplemental orders, perform chart audits, and assist with office duties as assigned. Provide end-of-life education and information to patients and families in compliance with CMS Conditions of Participation. Collaborate with the IDG to assess patient needs and coordinate care. Participate in performance improvement activities and orient/train new employees. ExpectationsExemplify Transitions LifeCare's Core Values in all interactions with patients, families, and coworkers. Minimum 2 years of recent clinical experience (Hospice, home health, or nursing home experience preferred). Hospice Nurse Certification preferred. Knowledge of Medicare regulations, licensing laws, and accreditation standards. Proficient in computer skills and CPR certified. Reliable transportation required. Must be able to travel and provide care in patient homes or facilities within the service area. Physical DemandsAbility to lift and/or carry up to 50 pounds. Ability to perform physical assessments and communicate effectively with patients, families, and physicians.
