Arrange non-medical support services (housing, transportation) to ensure treatment compliance. Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals. Engage specialty resources as needed to achieve optimal patient outcomes. Maintain detailed records of clinical, functional, and fiscal outcomes. Identify opportunities for health promotion and illness prevention. Intervene quickly to prevent adverse patient outcomes when possible. Serve as a patient advocate, ensuring privacy and confidentiality. Qualifications Associate's or Bachelor's Degree in Nursing or related field. Current, unrestricted RN license in the state of Minnesota.
