Position Summary: The Licensed Practical Nurse, under the direction and supervision of a Registered nurse, is responsible and accountable for the overall health status of their clients, including assessment, evaluation, plan of care, monitoring and reporting changes to the physician, RN, and family. The LPN may delegate selected nursing activities to CNA's but retains accountability and responsibility for the overall nursing care. Schedule: Rotating schedule of seven (7) days on and seven (7) days off, Wednesday through Tuesday. Twelve (12)-hour shifts, either 6:00 a.m.-6:00 p.m. or 6:00 p.m.-6:00 a.m. Job Duties and Responsibilities: 1. Initiates, performs and completes assigned duties in providing care to variable client populations 2. Observes, identifies, and responds to the client needs for care, including medication, equipment- assisted care and client/family education. In organizing and delivering care, the LPN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related factors. 3. Interacts in an appropriate manner with clients, family members, professional and other support personnel involved in the delivery of client care. 4. Provides effective education to clients and/or family members in relation to disease processes, medication and/or prescribed treatment regimens after collaboration with RN and/or health care team. 5. Documents client response to medication and/or prescribed treatment regimens and the reporting of any unexpected change in client condition to RN or provider. 6. Recognized urgent or emergent patient care situations, seek assistance of the RN and/or provider, and initiate appropriate emergency interventions as directed 7. Maintains adequate and timely records and reports of evaluation data, interventions, and client& response to interventions 8. Evaluates the effectiveness and outcomes of care provided 9. Practice of standard infection control precautions 10. Participates in agency quality management program measures 11. Participation in agency-directed in-service training programs as appropriate 12. Performs other related functions as required 13. Attends and participates in staff education and agency committee meetings as appropriate Minimum Qualifications: 1. Education and Licensure: a. Active unrestricted North Dakota LPN licensure in good standing b. Current BLS for Healthcare Providers 2. Experience: a. One (1) year of experience as a professional nurse within the last three years, with one (1) year of home care experience preferred. Knowledge, Skills, and Abilities: 1. Thorough knowledge and skill in current nursing practice. 2. Ability to teach and assess the competency of direct service providers 3. Knowledge of the concept of interdisciplinary teams 4. Knowledge of the development of individual service plans for client care and the ability to participate in them 5. Ability to recognize the needs and concerns of people, and to maintain constructive working relationships in dealing with them 6. Ability to communicate effectively both orally and in writing Additional Requirements: 1. Special certification as appropriate 2. Valid, current state driver license 3. Access to a reliable transportation Exposure/Risk Category: OSHA: Tasks involve occupational exposure to blood, body fluids or tissue. Compensation 30 per hour
